Before Your Visit
Before your appointment, please complete our intake forms in full, signing where necessary. Our intake forms are provided through PtEverywhere. You will receive emails from us describing how to access and complete these forms. You can access PtEverywhere through a web browser or by downloading an app for your mobile device.

We are an out-of-network (OON) provider. Insurance benefits vary, but you may be responsible for the entire cost of treatment. If you are unsure of your OON benefits please contact our office so that we can verify your benefits.
- The typical recommended initial course of treatment is between 4-6 treatment sessions.
- Every appointment is 1 hour one-on-one with a physical therapist, so that you receive the best possible care and expedite your recovery.
Dallas Frequently Asked Questions
Can I be receive treatment at IPA Physio Dallas without a prescription?
In Texas, patients have the right to seek treatment from a licensed physical therapist without a prescription or referral from a physician, saving you time and money. If you are in pain and have limited function, don’t delay starting PT. Let us help you begin your path to recovery! If we feel your condition needs further attention, we will recommend you see your physician, or a specialist, that can further evaluate you.
You can continue treatment with your physical therapist for a period of up to 30 calendar days, at which point you must obtain a prescription from a Medical Doctor (or other equivalent professional). For reimbursement, some insurance plans require a medical doctor’s prescription.
I haven’t been to IPA Physio Dallas in over 60 days. Why is another initial evaluation required if I am coming in for the same issue as before?
Can I get dry needling at IPA Physio Dallas?
Is trigger point dry needling the same as acupuncture?
I’m looking for Active Release Therapy (ART), do you provide this at IPA Physio Dallas?
